Admissions
|
|
Yes, registration staff can collect payment and issue a receipt.

Business Office
|
|
Insurance is billed as a courtesy, contact your insurance company for a claim status.
|
|
|
Contact the Business Office, we will conduct a chart audit to confirm that the service(s) were provided, and make necessary corrections to your bill if necessary.
|
Human Resources
|
|
HR post all open position every Friday in the Hospital. Positions are also available online.
|
|
|
Applications are available in the HR department from Mon-Fri 8 am-5 pm. Also available online.
|
|
|

Laboratory Services
|
|
No appointment is needed for out patient testing.
|
|
|
Most physicians will tell you if they want you to fast. If you are not sure, contact the laboratory.
|
|
|
Yes, you can as long as you have an order signed by your physician or health care practitioner.
|
|
|
In most cases, unless your doctor tells you to hold your medication, you should take them as prescribed. The exceptions to this is if you are on lanoxin (heart medication) and the doctor wants a digoxin level run. Blood for a digoxin level must be drawn at least 6 hours after the last dose.
|
Materials Management
|
|
Yes, you may check wilth us after 3:30 p.m. If we have boxes available, you are welcome to pick them up.
|
|
|
Boxes can be picked up for personal use by employees any day Monday through Friday after 3:30 p.m.
|
Medical Records
|
|
Come by the office and fill out a release for medical records, be sure to bring a photo I.D.
|
|
|
5 to 7 working days, depending on the number of requests preceding yours.
|
|
|
Not unless you indicate on the release form their name and we have their photo ID as well as yours.
|
|
|
It would depend on why you need the records. (Explanation: No charge for continuation of care, otherwise, there is a $15.00 charge to pull the chart and $.15 per page for every page we copy.)
|
|
|
Hospital policy does not allow us to fax any records to anyone unless a doctors office request a report because the patient is waiting in the office; secondly, we must have a signature to release any record.
|
|
|
We cannot release any records to anyone without the patient’s signature. If the patient is in Hospice, etc. we need a copy of power of attorney and a photo ID of the person retrieving the records.
|
|
|
In a minimum of four weeks patient can go to the county assessor’s office, request the certificate, the charge is $18.00. If for whatever reason the certificate is not in yet there will be another $18.00 charge every time they search for it until it comes in. Birth certificates are available on Mondays and Wednesdays only.
|
|
|
Social Security card will arrive by mail in 6-12 weeks.
|
|
|
You can use the babies and/or mothers hospital arm bands and hospital crib card for proof of birth. These items are accepted 98% of the time.
|
Pharmacy
|
|
The pharmacy only fills prescriptions for inpatients, the public and outpatients must have their prescriptions filled by their local pharmicist.
|
Physical Therapy
|
|
Our department accepts most insurance types. We can verify if we accept your particular insurance.
|
|
|
Yes, we are. As part of Palo Verde Hospital, we are a “Preferred Provider”, even though the PT Department is not listed separately as a provider on the insurance’s website. We operate under the umbrella of the hospital since we are employed by the Healthcare District.
|

|
It is impossible to predict what your out of pocket expense will be, unless you have a co-pay. If you do not have a co-pay, insurance companies will not disclose to us the necessary information to answer “how much”. We can refer you to a Business Office representative to further address your question.
|
|
|
If you want your insurance to pay for PT services, the answer is yes. If you wish to pay cash for services, then a prescription is not required since California is a Direct Access state. We handle these situations on a case by case basis.
|
|
|
Generally speaking, your evaluation and first treatment will take 60-90 minutes. Thereafter, plan on 60-75 minutes per session, depending on your condition.
